What is an Explanation of Benefits (EOB)?

An EOB is an account of how your insurance company processed your medical or pharmaceutical services. It will tell you what services were covered, which services were not covered, and what was paid for.

An EOB is NOT a bill.

The EOB is usually addressed to the insurance policy holder (ie: a parent if you are under their insurance plan).

What information is on my EOB?

An EOB will contain the date of service and list the services provided.

Services will be noted with:

  1. An ICD code
  2. A general description of the appointment
  3. Both 1 and 2

You can call your insurance provider and ask them specifically how the appointment information will be noted on the EOB.

If you prefer NOT to use your insurance for this or any other reason you can choose to be a Self-Pay patient.

Have EOB sent directly to you

If you are 18 or older and would like to have your EOB sent directly to you, as opposed to being sent to the insurance holder (who may be a parent/guardian/partner), call your insurance company and ask if this option is a possibility for your account. Some, but not all, insurance companies provide this option.

EOB's and Self-Pay

If you would like NO information sent home you can choose not to use your insurance and be a Self-Pay patient.

Self-Pay charges can be placed on your student account (Please Note: copays can NOT be billed to your student account) or paid at the time of service. The Health Center accepts cash, credit/debit card, Terp Express, and checks. Your student account bill will offer NO details of your Health Center visit; bills will only say “Health Center”.

If you prefer NOT to use your insurance for an appointment, please notify the Registration Desk at check-in. When you come for future appointments, you will be asked if you would like to continue Self-Pay or bill to insurance.

Insured Patient Costs

Patients may be able to use their health insurance for most medical services offered at the health center. Some services that the Health Center offers are Self-Pay only. Please contact your insurance provider for specifics on what services are covered. 

Self-Pay Costs

The Health Center also offers a Self-Pay option for patients who have out of network insurance plans or would rather pay out of pocket for services.

Below are Self-Pay prices for some services at the Health Center. Please be advised that prices are subject to change without notice and that some services are Self-Pay only. Patients should contact the Health Center if they have questions regarding costs.
