A confidential medical record is established and maintained for every patient who receives care. Separate records are maintained for Mental Health Services, Substance Abuse Programs. Information is released only with your written permission, upon a court ordered subpoena, or in a life threatening situation. If you are 18 years or over, information cannot be released even to your parents without your permission, except in aforementioned circumstances.
