2009-2010 Online Application for SHAC
Mission: SHAC serves as the student liaison to the University Health Center (UHC). SHAC is comprised of 10-15 students from a variety of majors. SHAC acts to ensure quality health care services by voicing student concerns to the UHC administration. SHAC also designs out-reach activities for the campus, such as blood drives, and serves on UHC committees that interview new staff.
Commitment: While SHAC is not extremely time-consuming, active participation at all SHAC meetings and events is expected of each member. SHAC meets for 1 hour every other week throughout the academic year; total time commitment is estimated at 15-20 hours per semester.
Please type your answers to the following questions. Please limit all of your responses to one hundred words per question. For best consideration, applications should be submitted in by April 17th, 2009 or dropped of at the Health Promotion Office on the ground floor of the University Health Center.
You must also sign up for an interview. The sign-up sheet will be posted from April 14 - 19, 2009 on the SHAC office door, Room 0139, on the Ground floor of the Health Center. Interviews will be held Apri 20-22.
