Complete applications (including signature) should be submitted to your supervisor who then would send it to the Healthy Workers Program. Do not drop off application in person to our office.

Once your application is approved, your HWP ID card will be sent to your supervisor.

Be sure to bring your staff photo ID when you come in for appointments.
