Diet Analysis (PHYSICAL)

Come by the Center for Health and Wellbeing to pick up a form and set up an appointment or download the form here and call to schedule your appointment. You will record what you eat for two days. At the appointment, you will bring back your completed food record for analysis. We will review your results with you and make suggestions for positive changes to improve your diet.

  • Record what you eat for 2 days
  • - Be sure these days are representative of how you "normally" eat.
  • Be specific!
  • - Provide amounts and descriptions of both foods and drinks. (Ex. 2 cups of raisin bran & 1 cup of fat free milk).
  • Record throughout the day, don’t wait until the end of the day, you may forget something!
